Emergency Response Protocol

When something urgent happens mid-sprint:

  1. Assess severity: Is it blocking current sprint work? Is it a production issue?
  2. Triage immediately: Don't wait for the next standup
  3. Create emergency ticket: Use priority P0, assign immediately

Multi-Agent Parallel Review (for major features)

After implementation, run /rev + /rob + /jorge + /all in parallel for maximum coverage:

Anti-Patterns to Avoid

  1. Passive facilitation: Don't wait to be asked — proactively trigger the next step
  2. Scrum Police: Over-enforcing rules without context
  3. Sprint Extension: Extending sprints to "finish" work
  4. Cherry-picking: Taking only easy stories
  5. No Retrospective: Skipping retros when "busy"
  6. Status Reporting: Turning standups into status meetings
  7. Scope Creep: Adding work mid-sprint without trade-offs
  8. Vague tickets: Creating tickets that require reading external reports (see Protocol)
  9. Skipping gates: Letting implementation start without /arch approval
  10. Forgetting knowledge capture: Not updating agent skills after sprint learnings
  11. Information silos: Not embedding expert outputs into tickets
  12. Blind delegation: Assigning work without checking if the agent has the needed context
  13. Big-bang delivery: Delivering everything at once instead of phasing into smaller, independently verifiable increments. Phase 0 (emergency/foundation), Phase 1 (refinement), Phase 2+ (enhancement) reduces risk and enables early QA feedback
  14. Ignoring condition folding: When multiple reviewers identify overlapping concerns (e.g., architect and security reviewer both flag the same area), fold them into existing tickets rather than creating duplicate tickets. Track the fold with source attribution ("Per /arch C5", "Per SecOps S2")
  15. Accepting investigation premises at face value: Always challenge the stakeholder's proposed solution before dispatching investigators. "Should we use X?" must be met with "Is X the right question?"
  16. Dispatching investigations without verification: Before assigning any investigation, verify the feature under analysis actually works. If broken, the investigation scope changes from "optimize" to "fix."

Copilot PR Review as Mandatory Gate

GitHub Copilot automated review should be a mandatory gate before merging PRs. In practice, it catches 5-10 real issues per PR that human reviewers miss:

  • Unused imports after refactoring
  • Missing HTML performance attributes
  • Hardcoded strings that should use model constants
  • Helper function logic bugs

Process: After /rev code review is complete and PR is created, wait for Copilot review comments. Fix all findings, push, and verify no new comments before merging.

Two-Phase Sprint Model — Validated

The two-phase sprint workflow works well:

  • Phase 1 (per-ticket): TDD implementation + /rev code review — keeps individual code quality high
  • Phase 2 (sprint-end batch): /adam writes E2E for the full integrated feature, /rob reviews, /rev reviews test code, then run tests

Benefits observed:

  • No fragmented E2E tests for incomplete features
  • Comprehensive integration testing of the full feature
  • Clear gates between phases

E2E Fix Round Baseline

For sprints with Filament admin + frontend E2E test suites, expect 3-4 fix rounds before reaching green. This is a stable baseline, not a failure signal. Common root causes by round:

  • Round 1: Seeder data issues (double-encoding, missing fields), wrong selectors from assumed HTML
  • Round 2: Filter/table state issues (default filters hiding records), missing confirmation dialogs
  • Round 3: Alpine.js visibility timing, modal transition races
  • Round 4: Edge cases in button targeting, scoping assertions to avoid related content sections

Improvement lever: Pre-discovering actual HTML with Browser MCP before writing tests reduces rounds by 1-2. The goal is to converge toward 2-3 rounds consistently.

Synthetic Test Data Pattern

For features that need specific database state for E2E testing (e.g., active ad campaigns), establish an artisan/CLI command for test data management:

  • seed action creates the minimum required data
  • cleanup action removes all synthetic data
  • Idempotent: safe to run multiple times
  • Flushes caches after changes

This pattern enables repeatable E2E testing without polluting production data. Consider integrating into test setup (beforeAll) for full automation.
